If this ext does not work just by installing it then it's time to find a real host and not a fly by night one plain and simple.

I changed PHP version to 5.3.29 and EVERYTHING WORKS JUST FINE, although files permissions are 0104 and after the upload there is no image dimensions. I guess errors we're experiencing are php related.

Open imageupload_body.html
Delete out whichever sections you do not want. See the attached image for more info.
Each coloured box represents the lines you would need to delete to remove one of the link options.

I have size restrictions on my posts, and so my users haven't liked this extention because they cannot use the display picture bbCode. I like it, but I was wondering if there would be a way to modify it so it displays a thumbnail instead of trying to display the full sized image with exceeds the size restrictions, or just a resize option on it.

I really liked how the old postimage.org website did their mod for the older version of phpBB where it would automatically close the upload popup, and with a JavaScript put the bbcode in the field for the user. That would be cool upgrade also.

Just some friendly suggestions, maybe you have already got something like that available?
