Unzip the downloaded release, and change the name of the folder to `formcreator`.
In the `ext` directory of your phpBB board, create a new directory named `dmzx` (if it does not already exist).
Copy the `formcreator` folder to `/ext/dmzx/` if done correctly, you'll have the main extension class at (your forum root)/ext/dmzx/formcreator/composer.json
Navigate in the ACP to `Customise -> Manage extensions`
Look for `Form Maker/Creator` under the Disabled Extensions list, and click its `Enable` link.

I previously had this on 3.1.7 and recently updated to 3.2 and also updated to the newest version of Form Maker and no longer see the "form" button in the posting a topic area. Any help would be appreciated.

It still shows my current version as 1.0.0-dev.
Did you add a form and select a forum in ACP?

Yes sir. I even deleted the entire thing and reinstalled it. It's weird because if I go to that forum and create a new topic, the button isn't there but, if I just hit preview without entering anything it will show up with the code. It also shows _ between each word.
